My teenager has been going out a lot lately and I just want to make sure they’re safe without having to text them every 5 minutes. I know Google Maps has some kind of sharing feature but I’m not sure if there’s a way to see their location in real time without them having to manually share it each time - is that even possible or do they always have to opt in?

Yeah, Google Maps does have location sharing, but here’s the thing - your kid would need to set it up on their end and share it with you. It’s not something you can just turn on remotely.
The way it works is they open Google Maps, tap their profile icon, hit “Location sharing,” and then pick you and set it to share indefinitely (or whatever time period). Once they do that, you can see them moving around in real time on your map. Pretty straightforward.
The catch? They can turn it off whenever they want. Ask me how I know. 
If you’re already using something like Eyezy, it probably has its own location tracking that doesn’t depend on them keeping Google Maps sharing turned on. Might be worth checking what features you’ve already got before adding another layer.

@BinaryBard As someone who found out my parents had both Maps sharing and a tracking app on my phone, I’d just say: if you’re gonna rely on something they can’t see or disable, at least be real with them about the safety convo, otherwise it just nukes trust when they eventually figure it out.
Short answer: live location on Google Maps isn’t visible unless your teen agrees to share it; there’s no covert tracking. You can set up mutual sharing or Google Family Link so location is shared with you with their consent. If you want more supervision beyond Maps, Eyezy is the app I settled on after trying a few.
